Early
mornings and late afternoons are reserved for short “out of school”
appointments. For initial bonding and banding appointments, we usually
need longer one-hour appointments in the morning. We always remain available
if you or your child has a problem or any questions. Please give notice
if you are unable to make your appointment.
Payment
Payment is due at the time services are rendered unless specific financial
arrangements have been made. Our office procedure is to have all accounts
paid within 30 days of service rendered.
Insurance
As a service to you, our office will submit charges for services to
your insurance company. However, we do consider the patient primarily
responsible for the account. When payment of insurance is assigned to
us, that portion of the fee payable by the patient is due at the time
of service.
Emergencies
We recognize that emergency situations do arise, and we will do our
best to respond promptly. If we cannot be reached at the office (303-421-9814),
please call Dr. Chapman at home (303-425-3282).
